Tetanus, also known as lockjaw, is a potentially life-threatening condition caused by the bacteria Clostridium tetani. This article presents a case report that highlights the challenges faced in addressing tetanus, emphasizing the importance of multidisciplinary involvement in its management. Case Presentation

In this case report, we present the details of a 35-year-old male patient who was admitted to our hospital with a suspected diagnosis of tetanus. The patient had a history of a minor foot injury a week ago, which he neglected to clean properly. He presented with muscle stiffness, difficulty in opening his mouth, and generalized spasms. Upon examination, his jaws were locked, and his neck and facial muscles were rigid.

Clinical Course

Following admission, the patient underwent a thorough evaluation to confirm the diagnosis of tetanus. Laboratory tests revealed elevated levels of muscle enzymes, indicating muscle damage. Cultures from the wound site confirmed the presence of Clostridium tetani. The patient was immediately started on a regimen of tetanus immunoglobulin and intravenous antibiotics to prevent the further spread of the bacteria.

Challenges in Treatment

The management of tetanus poses numerous challenges, requiring collaboration among various specialties, including infectious disease specialists, neurologists, intensivists, and nurses. Firstly, the extensive muscle stiffness and spasms seen in tetanus patients necessitate careful monitoring and supportive care to prevent complications like respiratory failure. In our case, the patient’s foot injury proved to be the site of entry for the bacteria, emphasizing the importance of wound management to eliminate the source of infection.

Multidisciplinary Involvement

The successful management of tetanus heavily relies on the involvement of multiple healthcare professionals from different fields. Infectious disease specialists play a vital role in initiating appropriate antibiotic therapy to control bacterial growth. They also work closely with pharmacists to determine the optimal dosing and duration of treatment, keeping in mind potential drug interactions and adverse effects.

Neurologists, being experts in the management of neurological disorders, are essential in assessing the patient’s neurological status, monitoring for complications, and adjusting treatment plans accordingly. They play a key role in identifying potential complications such as autonomic dysfunction, aspiration pneumonia, and cardiac arrhythmias associated with tetanus.

Intensivists bring their expertise in critical care medicine and are responsible for closely monitoring the patient’s vital signs, managing any hemodynamic instability, and providing mechanical ventilation support, if necessary. Their constant presence and collaboration with nursing staff ensure prompt initiation of life-saving interventions when needed.

Nurses, on their part, provide direct patient care, administering medications, and monitoring for any changes in the patient’s condition. They play a crucial role in promoting meticulous wound care, maintaining proper hygiene, and providing emotional support to patients and their families.


Tetanus is a severe infectious disease that requires a well-coordinated multidisciplinary approach for successful management. This case report highlights the challenges faced in addressing tetanus, emphasizing the importance of involving professionals from different medical specialties.
